The following checklist explains the steps and costs associated with the LifeSTAR program:

1. Call either Brent or Jacci and indicate that you'd like to sign up for the LifeSTAR "Getting Started Workshop."


2. You will be asked to attend a preliminary assessment/screening appointment. Each participant/couple must receive a one-time assessment with a LifeSTAR therapist before starting the program. Assessment includes (1) an addiction screening, (2) time for you to ask specific questions about your situation, and (3) final sign-up instructions and information about the workshop. Cost for the one-time screening is $100.00.


3. After completing the initial screening, you will be enrolled in the next "Getting Started Workshop." The workshop runs for six-weeks and includes over 18 hours of education. The workshop is designed to educate individuals struggling with pornography addiction AND their spouses. Treatment is more successful when couples attend together and become educated about addiction and recovery. Cost for Getting Started Workshop is $400 per participant ($800 for a couple). This price includes all workshop workbooks and materials. A discount of $50 will be given to couples who attend together. Please contact Brent or Jacci for details.


4. Payment for the "Getting Started Workshop" needs to be on or before the first day of the program. You can pay by check, cash, or credit card. You can make credit card payments online. Check or cash payments can be made in person at time of assessment or at the start of the first group.
