Private Practice Information
In addition to their roles as Co-Directors of LifeSTAR Oregon, Brent and Jacci also work with individuals, couples and families in their private practices. Brent has experience providing assistance for people struggling with depression, anxiety, obsessive compulsive disorders, sexual compulsive behaviors and marital and adolescent issues. Jacci has experience working with adolescents and adults with substance abuse and other addictions, depression, anger management, sexual abuse, health problems, parenting, and children and adolescents with behavioral problems.

Consultation and Training: Brent and Jacci are available for consultation or training on issues relating to sexual addiction and compulsion, internet safety in the home and educating parents on healthy sex education for children.

Speaking Engagements: Brent and Jacci are available for public speaking engagements. Feel free to contact either Brent and/or Jacci personally to discuss their availability if you are interested in having them come to your next meeting or group. Topics of expertise include sexual addiction, communication, parenting young children and adolescents, communication, and marital relationships.

Office Hours: Daytime, evenings and Saturdays. No Sunday appointments.

Insurance billing: Insurance companies will be billed as a courtesy. However, the client must check with the insurance company prior to your visit to confirm that there is coverage for services. Payment is expected at the time of service. It will be your responsibility to verify that your insurance company will remit reimbursement.

Fees:

• Initial Assessment Session (90 Minutes) $125.00
• Individual Session (55 Minutes) $115.00
• The fee for individual sessions may be reduced to $105.00 for client-pay situations, but requires pre-authorization

Payment: Clients are expected to pay for therapy services at the beginning of their scheduled session. Cash, personal check and credit cards are welcome. The fee for individual sessions will be reduced by $10.00 with cash/check payments.

Therapy Forms: All therapy forms will be emailed to you before your first scheduled appointment. Please print the forms, complete them and bring them to the assessment session. Forms include information regarding client rights, confidentiality, cancelling appointments, fees and details of the therapy process. If you do not have access to email, you may complete the forms in the office prior to your first appointment.
